Secondary and Revision Rhinoplasty: Frequently Asked Questions

Updated

In brief

Revision rhinoplasty is surgery on a nose that has already been operated on, usually done at least 12 months after the last operation. This page answers the most common questions about candidacy, grafts, recovery, cost and travel.

Key points

  • Revision usually waits at least 12 months after your last nose surgery.
  • Cartilage grafts from the septum, ear or rib are often needed.
  • Plan on about 10 to 14 days in Istanbul.
  • The shape keeps refining for 12 to 18 months.
  • The first step, an online assessment, is free.

These are the questions we hear most often from people considering revision rhinoplasty. If yours is not here, send it to us with your photos and we will answer it directly.

Candidacy and timing

Who is a candidate for revision rhinoplasty?

Anyone who has had a nose operation and is unhappy with the look or the breathing afterward may be a candidate. The nose should be healed enough to judge, and you should have realistic goals. Our surgeon decides after reviewing your photos and history, and may advise against surgery if the benefit is small.

How long should I wait after my last surgery?

Usually at least 12 months. Swelling in a nose that has been operated on settles slowly, and an early decision can lead to the wrong plan. Thick skin may need longer. Earlier surgery is considered only for a clear functional problem, and that choice belongs to the surgeon.

How do I know if my nose needs revision?

Swelling alone is not a reason, because it can last many months. Signs worth a closer look are a shape that has changed over time, persistent blocked breathing, visible asymmetry or a collapsing tip. A review of your photos can tell you whether the problem looks structural or is still part of healing.

Can I have revision if I had my first surgery abroad?

Yes. Many revision patients had their first operation in another country. Operative notes help a great deal, so ask your first surgeon for them. If you cannot get records, send what you remember, and our surgeon will plan from your photos and the examination.

Is a third or fourth operation possible?

Sometimes, but each operation leaves less tissue to work with and more scarring. Planning becomes more careful, grafts from the rib are more likely and the goals may need to be more modest. Our surgeon will tell you honestly whether another operation is a sound idea.

Surgery and grafts

How long does revision rhinoplasty take?

It usually takes 3 to 5 hours under general anesthesia, and most patients stay one night in hospital. The exact time depends on how much structure needs to be rebuilt and whether rib cartilage is harvested. Revision generally takes longer than a first operation.

Is the open approach always used?

Revision is usually done with the open approach, because it gives the surgeon a clear view of the scarred structures and makes grafts easier to place accurately. The small scar across the columella typically fades well. Your plan will state the approach.

Why would I need cartilage from my rib?

Rib cartilage provides the most material and strength. It is chosen when the septum is used up or when a large rebuild is needed, such as a collapsed bridge. It means a small scar at the chest and some soreness there during recovery. Ear or septal cartilage is used when it is enough.

Are grafts permanent, and can they warp?

Cartilage grafts are living tissue from your own body, and they usually last. Rib cartilage can warp or be partly absorbed in some patients, which is why surgeons carve and place it carefully. The risk is small but real, and our surgeon will explain it for your plan.

Can breathing be fixed at the same time as the shape?

Yes, and it often should be. Valve collapse, residual septal deviation and over-narrowing can be corrected with grafts and sutures during the same operation. We assess the airway with the appearance, so that one does not come at the cost of the other.

Recovery

How long is recovery after revision rhinoplasty?

The first stage takes about two weeks: the splint and stitches come out around day 7 to 10, and bruising is mostly gone by day 14. Most people return to desk work at about two weeks. Swelling lasts much longer, and the shape refines for 12 to 18 months, with the tip changing last.

Why does swelling last longer in revision?

Scar tissue from earlier surgery slows the drainage of fluid, and the tissues have already been through one healing process. As a result, revision swelling is slower to settle than in a first operation. The tip is usually the last area to refine, so patience is part of the plan.

When can I exercise again?

Walking is encouraged early. Avoid contact sport and heavy lifting for about six weeks, and ask your surgeon before restarting intense exercise. Your surgeon will give specific advice based on your grafts and how your nose is healing.

When can I wear glasses?

Avoid resting glasses on the bridge for about six weeks, and longer after a rib-graft bridge. Until then, lightweight options that do not touch the nose or tape-supported frames can help. Ask the team at your check-up what suits your situation.

What should make me call you?

Contact us if you have increasing pain after the first days, a fever, unusual redness or heat, a sudden change in shape or any bleeding that does not stop. Send photos and a short message on WhatsApp. It is better to ask early than to wait for the next check-in.

Cost and booking

How much does revision rhinoplasty cost?

Revision typically costs $5,000 to $8,500. The final quote comes only after our surgeon reviews your photos, because the price depends on the graft source, any functional work and overall complexity. The written quote confirms exactly what is included.

What does the package usually include?

It usually includes surgery, anesthesia, a hospital night, hotel, airport and clinic transfers, medication and follow-up. Details can vary between cases, so we confirm them in the written quote. Flights and personal spending are normally separate, and you should check anything unclear before you book.

Why is the quote given only after photos?

Revision cases differ widely. A tip repair with ear cartilage is a different operation from a bridge rebuild with rib cartilage. A quote given without seeing your nose would be a guess, so we review your photos and history first and give a price that matches the plan.

Is the online assessment free?

Yes. You send photos and your surgical details, our surgeon reviews them and you receive an opinion, a plan and a quote. There is no obligation to book. Many people take the plan home to compare it with other opinions.

How do I book surgery?

Once you accept the plan and agree a date with us, you are booked. Before that, we confirm that you have waited long enough since your last operation, and we explain how payment and the written terms work. We then help you organize the hotel and transfers.

Travel and aftercare

How long do I need to stay in Istanbul?

Plan on about 10 to 14 days. The splint and stitches usually come out around day 7 to 10, and our team wants to check your nose before you fly home. Staying until then lets us deal with early questions in person.

Is it safe to fly after surgery?

Flying is usually fine once your surgeon agrees, which is typically at the end of your stay. Keep your head up, avoid lifting luggage and follow the advice you are given. Tell us if you have any condition that affects long journeys, such as a history of blood clots.

What follow-up do I get at home?

You send photos over WhatsApp at set points during the first year, and our surgeon reviews your healing. If you have a concern between check-ins, write sooner. Follow-up cannot replace an in-person exam, which is why we plan it carefully before you travel.

What if I need help while I am away?

Contact us on WhatsApp straight away. For urgent symptoms such as heavy bleeding, high fever or trouble breathing, seek local emergency care and tell us afterward. Bring a copy of your operation notes, which a local doctor can use.

Can I bring someone with me?

Many patients travel with a partner, relative or friend for the first days after surgery. Please ask when you receive your quote, so the hotel and transfer arrangements can be written into it. Having someone with you can make the early days easier.

This page is general information written for patients. It does not replace a consultation or examination, and the right plan for your nose can only be set by a surgeon who has assessed you.
